Bio

Jonathan provides oversight for Johns Hopkins Medicine OPH clinical services teams, inclusive of population health care management, behavioral health, pharmacy, diabetes management, post-acute collaborative, as well as community health teams, and ensures services are evidence-based and effective. He leads the design and execution of OPH models of care to appropriate Johns Hopkins Medicine (JHM) and affiliated patients, clinicians, entities, and our communities. His clinical areas of interest include care continuum attribution management, value-based care, virtual care/remote patient monitoring, and alternative site delivery. Jonathan has a background in critical care nursing and acute operations, and previously served as a Corporate Director for Population Health, Post-Acute Services and Network at Bon Secours Mercy Health. Among his responsibilities, he had oversight of network design and referral management integration, developed episodic utilization management for attributed patients, and alternative site delivery in support of a cross-continuum model of care.  In past roles, he served as Corporate Director for Population Health Operations and Post-Acute Care at LifeBridge Health, as well as clinical leadership and operations roles at MedStar Health in Washington, D.C.. He holds multiple competencies in business / IT applications and performance improvement, a Master of Business Administration from James Madison University, a Bachelor of Science in Nursing from Niagara University and a Bachelor of Arts in Health and Human services from the State University of New York at Buffalo.
